Assessor: Deborah (assigned). Date still TBA. Approach: assess the person as if they were alone — discussion covered potential needs for transport, meal prep, and getting dressed. Family noted they already have a cleaner and gardener and are happy to keep paying for those themselves.

Home safety & recovery

Use walking frame (and knee brace). Watch for weakness; don’t over-exert but keep active. High protein / high energy diet. Stay hydrated. Rivastigmine patch stopped (off since hospital) — GP noted possible low blood sugar + dehydration link to drowsiness; neurologist to review.

Current Status • 27 July 2026

GP review 27 July 2026 (Dr Bruce Wakefield). Blood tests fine. Nerve conduction results from Dr Neil Simon not yet received — Bruce will chase. Thigh-weakness pathway may be the wrong track (uncertain until NCS report). Bruce suggested the rivastigmine (dementia) patch may have contributed to drowsiness/collapse episodes — can lower blood sugar, and with dehydration could cause drowsiness etc. Dad has been off the patch since the last hospital stay; remain off for now. Neurologist (Dr Mobbs) to follow up on this too once referred. Optional: 2 panadol three times a day for knee if wanted. Geriatrician Dr Margaret Beasley cancelled (family left cancellation message) — neurology/cognitive follow-up via Dr Rowena Mobbs after NCS results + GP referral. Good to proceed with melanoma excision 13 Aug NBH (Dr Shane O'Neill). Booked: cardiologist Dr Linda Lin 1 Sep 2026 2:00pm (Mona Vale, same day as Sue). Mobility: walking frame.
